Care Management

Care Managers find out what people need and want.  They try to match peoples choices and needs to what is available.

People may need support with e.g.

Care Managers can act as advocates and help people to speak for themselves.

Care Managers also look at the support people get and make sure it is right for them.  They may do this by telephone, by visiting or by having a review meeting.

Care Managers protect people who are vulnerable.  This may mean helping people who may be at risk of being hurt by others.
